Report a Claim

Prompt and accurate benefits are owed to an injured employee once their claim is determined to be compensable. You must report work-related accidents immediately to ensure proper delivery of benefits. Delay in these benefits may expose both you and LUBA to penalties and attorney fees.

1. Report of injury by telephone

This is the most preferred way to report a new claim. Call (888) 884-5822 or (225) 389-5822 and you will be connected to our intake specialist who gathers claim information.

We will put the information into our computer system and once completed, your new claim will go to the proper claims handler. This relieves you of the paperwork associated with reporting a new claim. We will complete all necessary forms and handle filings with the Office of Workers’ Compensation.

Information you will need when reporting the claim by phone includes:

  1. Employee name, Social Security number, date of birth and any other information regarding your  employee
  2. Date and facts of the accident
  3. Description of the injury or illness
  4. Details of any medical treatment that may have already taken place 

2. Report of injury by e-mail

Choose the appropriate state below, complete the form, save and email us at FROI@lubawc.com.  We will complete all necessary forms and handle the filings with the Office of Workers’ Compensation or respective commission.

*NOTE: IF NO RETURN TO WORK DATE IS LISTED THE CLAIM WILL BE CONSIDERED A LOST TIME CLAIM

3. Report of injury by fax or mail

You may also complete an employer report of injury/illness by fax. Choose the appropriate state above, complete the form and fax the report to (225) 389-9300, Attn: Claims Intake. We complete all necessary forms and handle the filings with the Office of Workers’ Compensation or respective Commission.

Timely reporting allows us to conduct a prompt investigation. Prompt reporting also enables us to control the claim from the beginning, which often saves employers’ money. Once our investigation is complete, we pay the claims we owe and defend the claims not owed under workers’ compensation. Timely reporting of every work-related accident saves you money!
