You'll qualify for a four day/four night vacation for two, to one of the nine listed destinations for every $50,000 (estimated annual premium at inception) that you write with LUBA Workers' Comp with effective dates of March 1, 2008 through December 31, 2008.
Only accounts over $10,000 (estimated annual premium at inception) will qualify. Each agency can win a maximum of 5 trips.
All premium accumulated during this contest also counts toward LUBA's 10 MOST WANTED TRIP.
By February 16, 2009, you will be notified of your qualifying sales activity. You must give 90-day advance notice of the chosen destination(s) and dates. Destinations must be claimed or taken between June 1, 2009 and November 1, 2010.
LUBA WORKERS' COMP is required by the IRS to provide unincorporated agencies with FORM 1099 for the value of the destination(s) chosen.
All travel award space is based on availability at the time of booking. All bookings are based on airlines, car rentals, hotels, tours and cruises of LUBA's choice. Upgrades, change penalties or cancellation penalties are at additional cost to the award winner.
